What is CFS/ME?

CFS/ME is a long-term illness affecting multiple body systems. Its hallmark feature is post-exertional malaise (PEM) — a significant worsening of symptoms following even minor physical or cognitive activity. This distinguishes it from general fatigue and means that "pushing through" activity typically makes the condition worse rather than better.

CFS/ME often follows a viral illness — including COVID-19 (Long COVID shares many features) — though the exact mechanism is not fully understood. It affects people of all ages and backgrounds and can range from mildly disabling to severely debilitating.

Symptoms

  • Profound, persistent fatigue lasting more than six months that is not relieved by rest
  • Post-exertional malaise — worsening of all symptoms after physical or mental effort
  • Unrefreshing sleep — waking feeling as tired as when you went to bed
  • Cognitive difficulties — memory problems, poor concentration, and "brain fog"
  • Orthostatic intolerance — symptoms worsen on standing or sitting upright
  • Widespread muscle or joint pain without swelling
  • Headaches, sore throat, and tender lymph nodes

Causes

The exact cause of CFS/ME remains under investigation. Current evidence points to abnormalities in immune function, energy metabolism, and autonomic nervous system regulation — often triggered by viral or bacterial infection. Genetic predisposition and prior psychological stress may also play a role.

See a doctor if you have had unexplained, disabling fatigue for more than three months — particularly if accompanied by post-exertional malaise.
