Obesity & Weight Management

Obesity is a complex chronic medical condition — not a lifestyle choice or a lack of willpower. It significantly increases the risk of type 2 diabetes, cardiovascular disease, joint problems, sleep apnoea, and certain cancers. A structured medical approach combining dietary guidance, exercise, behavioural support, and medication where appropriate supports sustainable, lasting change.

Understanding obesity

Obesity is defined as a BMI of 30 or above, though BMI has significant limitations — waist circumference, body composition, and the presence of metabolic complications give a more complete picture of risk. Obesity results from a complex interplay of genetics, environment, hormones, metabolism, medications, and psychological factors.

The prevailing narrative that obesity is simply a matter of eating less and moving more fails to account for the biological mechanisms that defend body weight and make sustained weight loss extremely difficult without structured support. Medical treatment addresses these mechanisms directly.

Health risks

  • Type 2 diabetes — obesity is the primary modifiable risk factor
  • Cardiovascular disease — hypertension, high cholesterol, and atherosclerosis
  • Obstructive sleep apnoea
  • Non-alcoholic fatty liver disease (NAFLD)
  • Osteoarthritis — particularly of the knees and hips
  • Certain cancers — including bowel, breast, endometrial, and oesophageal cancer
  • Mental health — depression, anxiety, and low self-esteem are significantly more prevalent

Causes & contributing factors

  • Genetic predisposition — family history is one of the strongest predictors of obesity
  • Hormonal factors — insulin resistance, leptin resistance, and hypothyroidism
  • Medications — corticosteroids, antidepressants, antipsychotics, and insulin promote weight gain
  • Sleep deprivation — disrupts hunger hormones and increases caloric intake
  • Stress and emotional eating — cortisol promotes fat storage, particularly abdominally
  • Obesogenic environment — highly processed food availability and sedentary lifestyles

When to seek medical help

Seek medical support if your weight is affecting your health, if you have tried to lose weight without sustained success, or if you have weight-related conditions such as diabetes, high blood pressure, or sleep apnoea. A medical approach provides access to the most effective tools — including medication — that are not available without a prescription.
