What is erectile dysfunction?

ED occurs when the physical or psychological mechanisms required for an erection are disrupted. It is not simply a sexual problem — ED is increasingly recognised as an important marker of overall cardiovascular health. In men under 50, new onset ED can be the first sign of underlying heart disease, diabetes, or hypertension that has not yet been diagnosed.

Speaking to a doctor about ED is an important step — not just to address the symptom, but to assess and protect your broader health.

Causes

Physical causes

  • Cardiovascular disease and reduced blood flow to the penis
  • Diabetes — damages both blood vessels and nerves involved in erection
  • High blood pressure and high cholesterol
  • Hormonal imbalances — low testosterone, thyroid disorders
  • Medications — antidepressants, antihypertensives, and others
  • Alcohol, smoking, and recreational drug use

Psychological causes

  • Anxiety — including performance anxiety — is one of the most common causes, particularly in younger men
  • Depression and low mood
  • Relationship difficulties and stress
  • Past sexual trauma

When to see a doctor

See a doctor if ED is persistent, recurring, or affecting your wellbeing and relationships. Do not feel embarrassed — our doctors approach this as they would any other medical condition: with professionalism and discretion. ED in younger men or ED that has developed suddenly warrants careful medical review to exclude underlying conditions.

Treatment options

  • PDE5 inhibitors (sildenafil/Viagra, tadalafil/Cialis) — highly effective first-line oral medication for most men with ED
  • Lifestyle changes — weight loss, exercise, reducing alcohol, and stopping smoking improve ED significantly
  • Treating underlying conditions — optimising blood pressure, blood glucose, or testosterone levels
  • Psychological therapy — CBT or sex therapy for performance anxiety, relationship issues, or psychogenic ED
  • Vacuum devices, penile injections, or implants — for cases not responding to oral medication
