Menopause

Menopause marks the end of a woman's menstrual cycles, typically occurring between 45 and 55. The years before (perimenopause) and after can bring significant symptoms including hot flushes, night sweats, mood changes, and sleep disturbance. Modern treatment — particularly HRT — is safe, effective, and can transform quality of life.

What is menopause?

Menopause is defined as 12 consecutive months without a menstrual period, marking the end of reproductive function. It is caused by the natural decline of oestrogen and progesterone production as the ovaries age. The average age of menopause in Europe is 51, though premature menopause (before 40) affects around 1% of women and requires prompt assessment and treatment.

The hormonal changes of menopause have far-reaching effects beyond symptoms — they increase the long-term risk of osteoporosis, cardiovascular disease, and cognitive decline. HRT, when started early, provides significant protection against all of these outcomes in addition to relieving symptoms.

Symptoms

  • Hot flushes — sudden waves of heat, flushing, and sweating; the most common symptom
  • Night sweats — disruptive sweating during sleep causing fatigue
  • Irregular periods — typically the first sign, during perimenopause
  • Mood changes — anxiety, low mood, irritability, and emotional sensitivity
  • Difficulty sleeping — often worsened by night sweats
  • Brain fog — difficulty with memory, concentration, and word-finding
  • Vaginal dryness and discomfort during sex (genitourinary syndrome of menopause)
  • Reduced libido
  • Joint aches and pains
  • Changes in skin, hair, and nail quality

Perimenopause

Perimenopause — the transition to menopause — can begin several years before periods stop. Hormonal fluctuations during this period can cause significant symptoms including irregular periods, hot flushes, mood changes, and sleep disruption, sometimes beginning in the mid-40s. Perimenopause is often overlooked or misdiagnosed as depression or anxiety. If you are in your 40s and experiencing these symptoms, a hormonal assessment is worthwhile.

When to see a doctor

Speak to a doctor if menopausal symptoms are affecting your quality of life, work, relationships, or sleep. You do not need to simply "put up with" symptoms — effective treatment is available and the benefits of HRT for most women significantly outweigh the risks.

Treatment & HRT

  • Hormone replacement therapy (HRT) — the most effective treatment for menopausal symptoms; modern body-identical HRT (oestradiol + micronised progesterone) has a very favourable safety profile for most women
  • Testosterone therapy — for women with reduced libido and energy not fully responding to oestrogen alone
  • Vaginal oestrogen — safe and effective for genitourinary symptoms, even for women who cannot take systemic HRT
  • Non-hormonal options — SSRIs, SNRIs, and gabapentin for women who prefer not to take or cannot take HRT
  • Lifestyle measures — regular weight-bearing exercise, a calcium-rich diet, and vitamin D supplementation to protect bone health

The "HRT causes cancer" narrative largely relates to older, now-outdated HRT formulations. Modern body-identical HRT is associated with minimal risk for most women and significant benefits — including protection against osteoporosis and cardiovascular disease. Our doctors will discuss the evidence and your individual risk profile at your consultation.
